All Care Medical collects and keeps personal health information so that you can be treated safely. This page explains what we collect, why, and what you can ask us to do with it.
What we collect
Your name and contact details, your Ontario health card number, your medical history and the medications you take, clinical notes from your visits, test and imaging results, referral and specialist correspondence, and a record of appointments.
Why we collect it
- To assess, diagnose and treat you, and to plan your ongoing care.
- To bill OHIP for insured services on your behalf.
- To contact you about appointments, results and follow-up.
- To meet the record-keeping obligations that apply to physicians in Ontario.
Who we share it with
Only where it is needed for your care or required by law: with laboratories and imaging services when tests are ordered, with specialists when you are referred, with pharmacies for prescriptions, with the Ministry of Health for billing insured services, and with public health where reporting is legally required. We do not sell personal information and we do not use it for advertising.
How it is kept
Records are held in the clinic's electronic medical record and in locked storage on site, with access limited to the people who need it to do their job. Records are kept for the period required of physicians in Ontario and then securely destroyed.
Your choices
- You can ask to see your record and to get a copy of it.
- You can ask us to correct anything that is wrong.
- You can ask how your information has been used or disclosed.
- You can withdraw consent for a use that is not required for your care or by law.
This website
This site does not set advertising cookies and does not track you across other sites. The map is embedded from Google Maps, which may set its own cookies when it loads. The booking widget and the contact form are demonstrations: they do not currently send or store anything.

This policy is a draft for the mockup. It should be reviewed against the clinic's actual record-keeping and software before the site goes live.
